Helping Hands, Orillia is now part of the Couchiching Ontario Health Team.
What is an Ontario Health Team?
Introduced by the provincial government in February 2019, Ontario Health Teams (OHT) is a new model of care that brings together health care providers to work as one team. The goal is to improve the coordination of care and services for patients and their families/caregivers across multiple care settings.
The members of this team, even if in different organizations or physical locations, will work together toward common goals for improving access to, and delivery of, local health care services. More information about Ontario Health Teams is available from the Ministry of Health.
Does this impact current services?
There will be no disruption to services. We are committed to our mission to support our clients’ ability to live independently and to maintain and enhance their quality of life. Working with our partners at the Couchiching Ontario Health Team will only enhance the delivery of healthcare services in our community.
We look forward to the new OHT model being in place and as it evolves, we will update our strategic plan accordingly. In the meantime, our dedicated staff and volunteers will continue to uphold our vision and values.
Our Vision: Embracing the ‘Journey of Aging’ with respect, care, and support. Bringing Independence Home.
Our Values: Caring, Accountability, Participation, Respect, Integrity
